Why this document matters
This form is utilized to ascertain the borrower’s preferred method of taking title to the property. The borrower would complete any non-borrowing names and email along with answering related questions. When Pylon orders title, upload this form to expedite the process. When the client orders title, forward this form to the borrower to confirm their preference. Note: This form is a template for information collection, not a validation form for qualification purposes.When this document is requested
This document is requested when:- Property purchase or refinance transaction
- Title order is being prepared
- Borrower needs to specify how they want to hold title
- Non-borrowing title holders are involved in the transaction
